fix(onset): a stuck Upstream lookup no longer hangs later requests; the CLI stops waiting after 2 minutes

Requests in one instance share an Upstream lookup per key. A lookup tied
to a request that's torn down may never finish, and every later request
for that project waited on it for good (a few reads in 13,000 under
load). Others now wait at most 15 s, then get "try again" (or the last
answer), and the next request starts a fresh lookup. The CLI gives a
host 2 minutes to start answering, not 15.
