feat(vcs): vcs log <path> and vcs blame, with a changed-path index

Each version keeps the paths it changed (folders added or removed whole
as the folder), worked out as it lands on the host and filled in once for
older versions. A path's history reads only the versions that touched it.
On 20,000 versions: vcs log <file> 10 ms (git 20-90 ms), vcs blame 40 ms
(git 210 ms). On the host, 29 s before the index, 0.1 s with it.
